Submit Final Evaluation

The Koch Foundation requires an evaluation of all funded projects within 30 days of the approved project end date. If funds are not spent within the approved project dates, then the grant recipient must request an extension from the Foundation and provide regular progress reports until such time as funds are expended.

While the Foundation prefers that the Final Evaluation Form be submitted electronically, we will also accept a copy sent by U.S. mail. If you prefer to submit a paper version, please click here to download the Final Evaluation Form in PDF format, and complete all required information. In this case, you do not need to submit the form online.
